    • Please fill out one order form per student, but feel free to combine payments for siblings.
    • Indicate the number of slices for each type of pizza you want and record the cost for each type of pizza.
    • Indicate the quantity of each fruit item you want and record the cost.
    • Please pay in cash for totals under $10.  Checks are fine for larger totals. We are not able to provide change; therefore, payment above your total will be considered a donation.
    • Please attach payment to form or enclose form(s) and payment in an envelope!
